* Delete SAFE_SLOTS_TO_IMPORT_OPTIMISTICALLY

* Update fork choice tests

* Remove TTD related flags

* Add deprecation warning

* Remove more dead code

* Delete EF on_merge_block tests

* Remove even more dead code

* Address Mac's review comments
